Student Services Forms
Application for Graduation - Submit this application at the beginning of your anticipated graduation semester for credit verification and timelydegree awarding and processing upon semester completion.
Course Overload Form (PDF) - If a student desires to register for course work in excess of 21 credit hours (excluding fast track courses), this form must be submitted for approval by the vice president of curriculum instruction.
Course Repetition Request - A student may be permitted to enroll in a curriculum course up to three times for credit, in order to improve a grade or replace a withdrawal grade
Credit by Exam (PDF) - If a student has proficiencies previously developed and would like to challenge the content of acourse to receive credit, this form will need to be submitted for approval.
Credit By Experience Form (PDF) - If a student would like to request approval for nontraditional educational experiences, this form must be submitted for approval.
FERPA Consent Form/Student Consent for Release of Records - The Family Education Rights and Privacy Act (FERPA) affords certain rights to students concerning the privacy of, and access to, their education records. Students may choose to complete and submit this form to the Registrar allowing the release of a student’s education records to specified third parties
Grade Forgiveness Request Form (PDF) - Students who return to the Johnston Community College after stopping out for a minimum of three years or 36 consecutive months prior to current term enrollment and wish to make a "fresh start" in pursuing educational goals may apply for the Forgiveness Policy.

Medical Withdrawal - Students may request a medical withdrawal based on injury, illness, or psychological/psychiatric disorder.
Religious or Solemn Observance (PDF) - Students requesting absence from class for religious or solemn observance must complete the Religious or Solemn Observance Absence Request Form and obtain instructor approval at least two weeks prior to the date of the absence. Students who miss class for religious or solemn observance will be granted the opportunity to make up work missed due to the absence.
Registration Drop/Add Form - (during registration periods and before 10% census date)
Registered students may drop or add a class within the published drop/add period.
The student must secure the advisor's approval on a drop/add form before the change
can be made official in the Records and Registration Department.
Student Reinstatement Form - A student may be dropped by an instructor due to excessive absences at any time during the semester, including that time period before the date designated each semester as the last day a student can withdraw. The student must obtain the permission of the instructor to reenter class.
Transcript Request - Students may request atranscript online through Parchment. Once the transcript order has been placed, an official transcript will be sent electronicallyor by mail to the institution, individuals, firms, or agencies per the student’s request. There is a $9 charge for each official and unofficial transcript.
Withdrawal (WD) Form - (after 10% census date and before the 65% of the course)
Student request to officially withdraw after the 10% census date of the course and
before the 65% point of the course. Credit hours attempted are not counted in computing
the GPA.
