Associate Degree Radiography Application (A45700)

The deadline to submit is March 1st at 5:00 P. M. of the admitting year. A cumulative grade point average of 2.0 or higher at the last college attended is required to submit the application form. If a high school graduate in the last ten (10) years with no prior college, a grade point average of 2.8 or higher is required. All admission requirements must have been met to be considered. A current application to the college must be on file.

The TEAS test (7th edition), must be completed before February 28th of the admitting year. A program application is required for all programs you are nterested in applying to.

Applicants not completing the TEAS at JCC must - or if taken virtually/on-line, or at another College/Unviersity - must request that ATI send an offical copy to the Health Sciences Student Navigator at jcchealthprograms@johnstoncc.edu

If applying to multiple programs, please indicate your preference by selecting a program of study for your 2nd and 3rd choice. Radiography being your 1st Choice.

English: Did you place into ENG 111 – Writing & Inquiry
without co-req?

Place into ENG 111 without co-req.
Place into ENG 111 with co-requisite (011).
(If completed or enrolled in spring of application year they can take it with the required corequisite. Applicants can apply. Must complete co-requisite to be eligible for final consideration for the program with a grade of “C” or better.)
Sciences: Sciences (2 sciences total) Must be completed by March 1st of the admitting year. If a student has 2 high school science classes (Biology, Chemistry, or Physics) with a “C” or better you will not need college level Biology and Chemistry.

Note:

*Anatomy and physiology will not meet this requirement.
*CHM 090 will no longer be accepted.

Additional Coursework:
Courses required in the Radiography Program. Courses
may be taken prior to entering the program. Eligible for points if completed with “A” or “B” or “C”grade by March 1 of admitting year. Points ranking is completed by the College.

Points awarded as follows:
A=4 pts, B=3 pts, C=2 pts, A & P: A=8 pts, B=6 pts, C=4
pts, (Optional) Job Shadowing or CNA = 6 pts.

Applicants who have completed a Nurse Aide class must hold a current listing on the North Carolina Nurse Aide registry and completed an approved CNA class to receive points.

Optional:
Note: MED 120-Survey of Medical Terminology (preferred)
or MED 121-MedicalTerminology I or a college level Medical
Terminology class.

Job Shadowing or CNA certification is optional but will
provide additional points on the point ranking.

Applicants who have completed a Nurse Aide class must
hold a current listing on the North Carolina Nurse Aide
registry and completed an approved CNA class to receive
points.

* Copy of the CNA registry listing.is required to be provided.

Job Shadowing form (can be obtained by the Dept. Chair of
Imaging). Please provide the completed form with the
completed application.
Applicants must have met the admission requirements to apply to the Associate Degree Radiography program by the deadline date of (3/1st-5pm). Application packet must be complete.
