Intent to Apply Form - Medical/Cardiovascular Sonography

The deadline to submit is March 1st at 5:00 P. M. of the admitting year. A cumulative grade point average of 2.8 or higher at the last college attended is required to submit the application form. All admission requirements must have been met to be considered. A current application to the college must be on file. The TEAS test (Version VII), must be completed before February 28th of the admitting year. A program application is required for all programs you are interested in applying to.

Note: Applicants are responsible to ensure all required documents are on file. Incomplete packets cannot be processed. Applicants not completing the TEAS at JCC must request TEAS scores 7th Edition from ATI directly. Please view their web-site at www.atitesting.com for additional information. A score of exemplary or advanced or proficient on the TEAS and have a 2.8 grade point average or higher to meet final consideration requirements. Prerequisite Coursework Must complete or be enrolled in final pre-requisites when applying to the Sonography program. Courses must be completed by May 15th of the admitting year with a minimum grade of “C” or better. For Courses not completed at Johnston Community College, final transcripts must be on file in The Enrollment and Student Success Office by May 15th of the admitting year.
