PDF Fillable Form Help
Some forms on this site may be filled-in on your computer and
printed, or printed and filled-in by hand. Forms completed on your
computer are not submitted electronically. You must print the
completed form and submit it by mail, fax, or in-person.
If you choose to complete your form on your computer, you may need to know the following:
- If you are using Adobe Acrobat Reader, you cannot save the
text you enter in a form. When you close the form, all data you have
added will be cleared. You must print the form prior to closing
it. You may save a completed form if you have Adobe Acrobat Approval
or Adobe Acrobat (not Reader).
- You may magnify the form to make it easier to read by
changing the zoom level in the box in the lower left corner.
- To start entering text in the form, make sure the Hand Tool
is selected and click in the first field. To move from one field to
the next, press the "Tab" key. Holding down the "Shift"
key while pressing the "Tab" key will reverse direction of tab. We recommend that you use the
"Tab" key so that you will not skip over any fields. You may
also use your mouse to move from field to field.
- Some fields contain multi-line text boxes that should
automatically wrap within the box. You may also move to the next
line within a multi-line field by pressing the "Enter" key. You may not
be able to see all of the text in this field until you have tabbed to the
next field.
- Once a field is full, any extra characters entered will not be
visible on the form. You may need to fill these in by hand after
printing.
- Please note - Prior to printing your form, click the
hand icon anywhere outside of a fillable area. If a field contains a
blinking cursor, the contents of that field will not print.
- If the fillable form is displayed within your browser’s
window, be sure to use the printer button on the Acrobat toolbar menu to
print the form instead of your web browser’s print function.
